Practice Delivery Results Form

Please complete and submit this form following completion of your Practice Delivery Sessions. You must deliver at least 3 sessions and all seven sessions if you are learning to facilitate the COPE program in your advanced nursing course. Following a review of the information provided, your program delivery license, a parent letter explaining the program, and a COPE evaluation form will be sent to you electronically (usually within 72 hours).
